About Community Provider Network
Community Provider Network is one of four divisions within Community Health System that focuses on supporting providers. Community Provider Network (CPN) represents the continued effort to provide a wide range of healthcare services, optimize resources for our provider partners and ultimately serve our patients better.
There are three entities within CPN: Community Health Partners, Community Provider Plus (MSO) and Community Collaborative Care Network (CCO).
